   *sa - salutation
   *fn - first name
   *ln - last name
   *ti - title
   *co - company name
   *a1 - address line 1
   *a2 - address line 2
   *st - two letter abbrev for state
   *ST - spelled out state
   *ci - city name
   *zi - zip code

   *po - phone one
   *pt - phone two

   *me - memo field

   *u1 - user defined index field 1

This is an example of justification :

first name RIGHT justified padded with space

Printer code example:

p0 corresponds to printer code 0 which is setup in the PRINTER
PARAMETER MENU.  Insert these printer codes anywhere in your 
custom form.  The printer codes already installed are setup 
for the IBM Proprinter/Epson printers.
