<P>现代医学研究对此病主要有以下五方面的说法： <BR> a) 子宫内膜种植学说：Sampson最早提出月经期脱落的子宫内膜碎片，随经血逆流经输卵管进入腹腔，种植于卵巢和邻近的盆腔腹膜，并继发生长和蔓延，发展成子宫内膜异位症。有生殖道畸形或梗阻的妇女常并发子宫内膜异位症，说明经血逆流可致子宫内膜种植。腹壁刀口子宫内膜异位或分娩后会阴伤口出现子宫内膜异位症，是手术者将子宫内膜带至切口造成医源性种植。 </P>
<P> b) 淋巴及静脉播散学说：在盆腔静脉或淋巴结中发现子宫内膜组织存在支持该论点。并认为远离盆腔部位的器官如肺、手、大腿的皮肤和肌肉发生的子宫内膜异位可能是淋巴或静脉播散的结果。</P>
<P> c) 体腔上皮化生学说：卵巢的生发上皮、盆腔腹膜、胸膜均起源于体腔上皮，反复受经血、激素或慢性炎症的刺激可以化生为子宫内膜样组织，形成子宫内膜异位症。 </P>
<P> d) 免疫学说：有人认为在妇女免疫功能正常的情况下，月经期经输卵管流入腹腔的内膜细胞为主的局部免疫系统所杀灭，若局部免疫功能不足或逆流腹腔内的内膜细胞数量过多时，免疫细胞不足以将杀灭，即发生子宫内膜异位症。也有报道子宫内膜异位症患者有红斑狼疮或其它自身免疫疾病史者为无该病患者的两倍。从实验结果表明，在子宫内膜异位症患者血清中lgG及抗子宫内膜自身抗体较对照组明显增加，其子宫内膜中的lgG及补体C3沉积率亦高于正常妇女，故认为子宫内膜异位症可能是一种自身免疫性疾病。目前认为子宫内膜异位症患者既可有体液免疫，即B细胞应答反应增强，亦可有细胞免疫，即T 细胞免疫功能不足。上述免疫功能的异常是内膜异位的原因，还是内膜异位的结果仍有待确定。 </P>
<P> e) 遗传因素：Simpson提出子宫内膜异位症患者一级亲代（女性）中患有同病者，明显高于对照组，然而并未有发现与该病相关的特异性HLA抗原存在。 </P>
