<p><font face="Arial" color="#000000">(一)病因</font></p>
<p><font face="Arial" color="#000000">细菌、病毒、真核和原核微生物、原虫等经外阴阴道上行蔓延可导致子宫内膜炎，继而影响到输卵管、卵巢和盆腔腹膜。患内膜炎时，局部炎性细胞浸润和炎性介质渗出，对胚胎有毒害作用，不利于精子存活和孕卵着床。另外，病原体所呈现的抗原作用可激活机体的细胞及体液免疫反应，产生大量致敏的活性细胞，并产生多种细胞因子、炎性细胞，可杀灭和吞噬精子，免疫抗体可通过干扰正常胚胎和内膜间的组织相容性而影响孕卵着床、胎盘植入和胚胎发育。此外，严重的感染可破坏内膜的完整性和功能，引起月经不调和不孕。</font></p>
<p><font face="Arial" color="#000000">常见的病原体包括葡萄球菌、链球菌、厌氧菌、衣原体、支原体、弓形体、滴虫、阿米巴原虫、梅毒螺旋体、真菌、结核等。值得注重的是病毒感染的病例逐渐增多，且病毒性宫内感染可垂直传播给胎儿，造成早产、流产和胎儿畸形。</font></p>
<p><font face="Arial" color="#000000">(二)临床表现</font></p>
<p><font face="Arial" color="#000000">急性子宫内膜炎起病较急，多有明显诱因，如经期不卫生、经期不洁性交、宫腔操作、阑尾炎和全身感染等。表现为寒热，发烧(38～40℃)，全身无力，下腹剧痛、下坠，腰酸，大量血性、脓性或水样白带，并有恶臭。患者下腹压痛，宫颈举痛，宫休柔软胀大，压痛明显。</font></p>
<p><font face="Arial" color="#000000">由于宫腔有良好的引流条件及周期性内膜剥脱，使炎症极少有机会长期存在于内膜，但如急性期治疗不彻底，或经常仍存在感染原，则可导致慢性子宫内膜炎。临床上最常见的不孕因素是慢性结核性内膜炎和子宫内膜息肉，可表现为原发或继发性不孕，月经失调，白带增多，下腹坠痛。轻者双合诊可无异常发现，若有宫腔积脓，则子宫呈球状增大，柔软压痛，可见血性脓液自颈管排出，常并存急性阴道炎。</font></p>
<p><font face="Arial" color="#000000">(三)诊断</font></p>
<p><font face="Arial" color="#000000">据病史、症状和体征并不难诊断，结合对阴道、宫颈和宫腔分泌物行细胞学、细菌学和其他病原体检查，可发现病原体类型：行B超、HsG、官腔镜等检查可了解宫腔内病变范围及程度；诊断性刮宫可了解内膜组织学变化，如内膜结核、内膜息肉等。</font></p>
<p><font face="Arial" color="#000000">(四)治疗</font></p>
<p><font face="Arial" color="#000000">(1)若有明显诱因，则将其去除。</font></p>
<p><font face="Arial" color="#000000">(2)抗生素：针对病原体和药敏实验选择敏感抗生素，必要时联合用药。子宫内膜炎以全身治疗为主。对于慢性内膜炎、颈管炎有粘连、积脓者，应行颈管扩张、引流及宫腔抗生素注药或低压灌注。</font></p>
<p><font face="Arial" color="#000000">(3)对于子宫内膜息肉，可行直视下、宫腔镜下或手术切除。对于发生宫颈管或宫腔粘着者，应行宫颈扩张或宫腔镜下粘连分解术。</font></p>
<p><font face="Arial" color="#000000">(五)预后</font></p>
<p><font face="Arial" color="#000000">经及时有效的治疗可使多数感染完全恢复，不影响生育。但当同时患有慢性宫颈炎、内膜炎、子宫肌炎、附件炎或盆腔结缔组织炎时，治疗效果差，可导致不孕。<br />
