                        卵巢最主要的功能是产生卵子。发育良好的卵泡和准确的排卵日期为优生优育奠定了基础。在不孕症的诊断过程中，常常需要监测卵泡，目的是看看有没有正常的卵巢生长、发育、排出等。这种检查看来比较麻烦，但非常重要，具有重要的临床意义。
<p>　　<strong>检查方法</strong>：一般情况下，自月经周期的第8天起，每2天B超监测1次，当发现卵泡直径达17毫米时，应该改每天监测1次，当卵泡发育成熟直径达20-23毫米时，必要时每天测2次，直至排卵为止。B超探头频率3.5MHz每次监测，观察双侧卵巢大小、形态，记录卵泡数量、大小，月经第几天等。</p>
<p>　　应用B超监测排卵,对于月经周期规则、基础体温双相的不孕妇女，往往以为卵泡发育正常而忽视对其进行进一步检查，实际上，这类病例中卵泡发育和排卵异常是比较多见的，运用B超监测育龄妇女,特别是不孕妇女卵泡发育详细情况，确认其有否卵泡发育成熟、有否排卵及排卵时间，为临床对症治疗提供可靠依据是十分重要的。</p>
<p>　　有<strong>排卵周期</strong>的卵巢及卵泡表现。成熟卵泡的特征，B超监测最大卵泡出现在月经10-16天，卵泡直径20mm，呈圆形，壁薄，向卵巢的一侧突出，内部透声好,一般在10小时之内排卵，排卵时间在月经第12-18天内为多见，少数可在20-30天。排卵后特征:卵泡壁部分塌陷，边缘皱缩呈锯齿状，内有许多细弱光点，子宫直肠隐窝内可见液性暗区5-15mm，一般在排卵1天后消失，在此前后时间同房怀孕成功率高达90%。</p>
<p>　　不排卵周期的卵泡。黄体化卵泡不破型比较常见,B超监测见优势卵泡形成后，卵泡继续增大，卵泡最大者可达50-60毫米,包膜由薄渐厚，界限渐模糊，囊泡张力减低，囊内渐变为不均匀光点回声，一般情况下，直到下次月经周期中消失。但也有少数患者持续存在3个月后消失。</p>
<p>　　在运用B超监测排卵，指导不孕症患者临床用药，具有操作简便、安全无创、实用有效，可连续监测，找出病因，动态观察卵泡生长发育成熟情况，准确预测排卵等优点。对优势卵泡发育不全及排卵延缓或形成卵泡囊肿者，一般在B超连续监测过程中，能发现一些与激素水平变化不一致的特殊情况，根据超声的图像特征可以判断卵泡的成熟度和是否排卵。所以B超对卵泡发育的监测，能准确预测排卵日期，指导临床用药及性生活时间，提高不孕者的受孕率，对不孕症的治疗，具有实用的临床应用价值。</p>
