<DIV class=wenz-text><FONT face=Arial color=#000000> 高频电波刀的电圈切除术（LEEP）是1981年法国学者首次报道的。现已广泛应用于临床， LEEP手术的另一个提法为“大环状宫颈移行带切除术” </FONT></DIV><FONT face=Arial color=#000000>
<DIV class=wenz-text><BR> 一、 高频电波刀的原理：</DIV>
<DIV class=wenz-text><BR> 高频电波刀是采用高频无线电刀通LOOP金属丝由电极尖端产生3.8兆赫的超高频（微波）电波，在接触身体组织的瞬间，由组织本身产生阻抗，吸收电波产生高热，使细胞内水分形成蒸汽波来完成各种切割、止血等手术目的，但不影响切口边缘组织的病理学检查。高频电波刀与传统电刀的原理是不同的：传统电刀是由电极本身阻抗，因电流通过而产生高热来达到手术目的，输出频率是0.3-1.0兆赫，而高频电波刀射频转化的热能产生于组织内部，由射频产生正弦波使细胞内水分震荡，产热蒸发，发射术所接触的细胞破裂从而使组织分开，而射频发射极本身不发热。</FONT></DIV><FONT face=Arial color=#000000>
<DIV class=wenz-text><BR> 二、 高频电波刀的优点：</DIV>
<DIV class=wenz-text><BR> （1） 高频电波刀是由多种电极组成的，包括环形、球形、针形、三角形、方形等。<BR> （2） 包括射频技术所有的功能：切割、凝血、消融、蒸发、清楚、收缩、电灼（包括激光和电刀的所有功能，且没有他们的副作用）；<BR> （3） 手术精确，可以达到传统电刀达不到的非常精细的手术效果；<BR> （4） 微创：无压力切割，组织损伤小（损伤深度小于20微米，很少有纤维形成，术后很少发生疤痕）；<BR> （5） 很不发生传统电刀所造成的组织拉扯、碳化的现象，约2/3的病人可以得到不影响病理检查的组织标本。<BR> （6） 痛苦小，不用麻醉或仅用局部浸润麻醉，并发症少（出血和感染少）；<BR> （7） 安全：不需要负极和地线，无触电及烧灼的危险；<BR> （8） 对手术室无特殊的要求，可在门诊进行；</FONT></DIV><FONT face=Arial color=#000000>
<DIV class=wenz-text><BR> 三、 LEEP手术的适应症：</DIV>
<DIV class=wenz-text><BR> （1） 慢性宫颈炎：宫颈糜烂、宫颈外翻、宫颈白斑、宫颈息肉（颈管内大的息肉或多发息肉）；<BR> （2） 宫颈湿疣，亚临床HPV感染可用药物治疗，但如果HPV-16、HPV-18型或合并CIN，或病变范围较大，无条件随访者，应行物理治疗或LEEP治疗；<BR> （3） CIN；怀疑CIN2或CIN3：持续CIN1或CIN1随访不便的患者。CIN3包括重订非典型增生和原位癌，而LEEP只适用于重度非典型增生，但不宜用于宫颈原位癌，LEEP治疗原位癌后的复发率是29%，而传的冷刀治疗后的复发率是6%。<BR> （4） 怀疑宫颈非典型鳞状细胞</FONT> </DIV>
