<p><font face="Arial" color="#000000"> 下丘脑—垂体—卵巢性腺轴任何部位异常，均可导致不排卵。有一些检查方法，能够确定性腺轴的异常部位。明确性腺轴的异常部位，对治疗有一定的知道指导意义，因为性腺轴异常部位的不同，用药种类与方法也不同。 </font></p>
<p><font face="Arial" color="#000000"> （一）、内分泌测定： </font></p>
<p><font face="Arial" color="#000000"> 1．泌乳素升高： </font></p>
<p><font face="Arial" color="#000000"> 正常育龄妇女血清PRL值为5—25ng/ml，如果>25ng/ml即为升高。高水平的PRL对下丘脑—垂体—卵巢性腺轴功能起抑制作用，可能的机理为：①抑制下丘脑GnRH的合成与释放；②抑制雌激素对下丘脑、垂体的正反馈；③对卵巢的直接抑制作用。PRL>60ng/ml，就有可能发生闭经。 </font></p>
<p><font face="Arial" color="#000000"> 检测血清PRL水平，采血一般在早晨9—10点、空腹、安静状态下进行。当PRL>100ng/ml时，应常规行蝶鞍CT或MRT检查，以排除垂体腺瘤。因为血清PRL>50ng/ml时，约20%为垂体瘤；血清PRL>100ng/ml时，垂体瘤的可能性为50%；血清PRL>200ng/ml则几乎100%为垂体瘤； </font></p>
<p><font face="Arial" color="#000000"> 治疗：溴隐亭、手术。 </font></p>
<p><font face="Arial" color="#000000"> 2．低促性腺激素状态： </font></p>
<p><font face="Arial" color="#000000"> 正常生育年龄妇女血清FSH与LH为5—40IU/L，如果FSH与LH<5IU/L，为低促性腺激素性排卵障碍，表明病变在下丘脑或垂体，功能障碍，例如席汉氏综合征。治疗：GnRHa,hMG,rFSH. </font></p>
<p><font face="Arial" color="#000000"> 3．高促性腺激素状态 </font></p>
<p><font face="Arial" color="#000000"> 血清FSH与LH>40IU/L,为高促性腺激素性排卵障碍，表明病变在卵巢，如卵巢早衰、性腺发育不全等。治疗：赠卵试管婴儿。</font></p>
<p><font face="Arial" color="#000000"> 4．LH/FSH>3、T升高，为多囊卵巢综合征。治疗：二甲双胍、克罗米芬hMG,rFSH.等。 </font></p>
<p><font face="Arial" color="#000000"> 5．T3、T4、TSH，及尿17-羟、17-酮测定，排除甲状腺及肾上腺病变。 </font></p>
<p><font face="Arial" color="#000000"> （二）、黄体酮试验 </font></p>
<p><font face="Arial" color="#000000"> 方法：黄体酮20mg/日、3-5天，停药2-10观察。 </font></p>
<p><font face="Arial" color="#000000"> 阳性：有撤药性出血，表明体内有雌激素，宫内膜呈增殖期改变，不排卵部位在下丘脑或垂体。治疗：克罗米芬hMG,rFSH.等。 </font></p>
<p><font face="Arial" color="#000000"> 阴性：无撤药性出血，说明体内缺乏雌激素，病变可能在卵巢或下丘脑及垂体。 </font></p>
<p><font face="Arial" color="#000000"> （三）、尿促性素（hMG）试验： </font></p>
<p><font face="Arial" color="#000000"> 对黄体酮试验阴性者行HMG试验，以确定不排卵部位在卵巢本身或之上。方法：hMG150单位注射，每日1次，5—7天，超声观察有否卵泡发育及排卵。 </font></p>
<p><font face="Arial" color="#000000"> 阳性：有卵泡发育或排卵，病变在垂体或下丘脑。治疗：hMG,rFSH. </font></p>
<p><font face="Arial" color="#000000"> 阴性：无反应，病变在卵巢本身。疗效差。 </font></p>
<p><font face="Arial" color="#000000"> （四）、垂体兴奋试验： </font></p>
<p><font face="Arial" color="#000000"> 方法：静脉注射GnRH100ug,注前及注射后15、30、60、120分各抽血一次，测LH水平。 </font></p>
<p><font face="Arial" color="#000000"> 阳性：与注射前比较，注射GnRH后30-60分，LH上升3倍。表明垂体反应良好，病变在下丘脑。治疗：GnRH。 </font></p>
<p><font face="Arial" color="#000000"> 阴性：与注射前比较，注射GnRH后LH水平无明显变化。病变在垂体。治疗：hMG,rFSH. </font></p>
<p><font face="Arial" color="#000000"> 垂体兴奋试验的局限性：垂体虽然有病变，但未完全被破坏，仍可释放较多LH；有时垂体正常，但由于较长时间未受内源GnRH刺激，有一定惰性，一次垂体兴奋试验可能出现阴性反应。 </font></p>
<p><font face="Arial" color="#000000"> （五）、克罗米芬（CC）试验： </font></p>
<p><font face="Arial" color="#000000"> CC100mg、每日1次，连服天，用药前及停药5天测血清FSH及LH水平，如用药后FSH及LH升高3—10倍为阳性反应，表明下丘脑—垂体功能正常（病变可能在卵巢）；如果无明显变化，为阴性反应，病变在下丘脑。<br />
