<p><font face="Arial" color="#000000"> 导内乳头状瘤临床上常与囊性增生病、导管扩张综合征、浸润性导管癌、导管内乳头状癌相混淆，要综合分析，仔细辨证而确定。</font></p>
<p><font face="Arial" color="#000000"> 导管内乳头状瘤，中医称之为“乳衄”，是由于忧思过度，肝脾两伤，肝气郁结，气滞血瘀，瘀血阻络到局部肿块。肝气郁久化热，热久化火，肝火亢盛，肝不藏血，脾气受损，脾不统血，血失统藏，迫血妄行而导致。<br />
乳腺导管内乳头状瘤需与乳腺导管内乳头状癌及乳腺导管扩张综合征相鉴别。</font></p>
<p><font face="Arial" color="#000000"> 1）乳腺导管内乳头状瘤与乳腺导管内乳头状癌：两者均可见到自发的、无痛性乳头血性溢液；均可扪及乳晕部肿块，且按压该肿块时可自乳管开口处溢出血性液体。由于两者的临床表现及形态学特征都非常相似，故两者的鉴别诊断十分困难。一般认为，乳腺导管内乳头状瘤的溢液可为血性，亦可为浆液血性或浆液性；而乳头状癌的溢液则以血性者为多见，且多为单侧单孔。乳头状瘤的肿块多位于乳晕区，质地较软，肿块一般不大于 1cm，同侧腋窝淋巴结无肿大；而乳头状癌的肿块多位于乳晕区以外，质地硬，表面不光滑，活动度差，易与皮肤粘连，肿块一般大于1cm，同侧腋窝可见肿大的淋巴结。乳腺导管造影显示导管突然中断，断端呈光滑杯口状，近侧导管显示明显扩张，有时为圆形或卵圆形充盈缺损，导管柔软、光整者，多为导管内乳头状瘤；若断端不整齐，近侧导管轻度扩张，扭曲，排列紊乱，充盈缺损或完全性阻塞，导管失去自然柔软度而变得僵硬等，则多为导管内癌。溢液涂片细胞学检查乳头状癌可找到癌细胞。最终确立诊断则以病理诊断为准，而且应做石蜡切片，避免因冰冻切片的局限性造成假阴性或假阳性结果。</font></p>
<p><font face="Arial" color="#000000"> 2）乳腺导管内乳头状瘤与乳腺导管扩张综合征：导管内乳头状瘤与导管扩张综合征的溢液期均可以乳头溢液为主要症状，但导管扩张综合征常伴有先天性乳头凹陷，溢液多为双侧多孔，性状可呈水样、乳汁样、浆液样、脓血性或血性；乳头状瘤与导管扩张综合征的肿块期均可见到乳晕下肿块，但后者的肿块常较前者为大，且肿块形状不规则，质地硬韧，可与皮肤粘连，常发生红肿疼痛，后期可发生溃破而流脓。导管扩张综合征还可见患侧腋窝淋巴结肿大、压痛。乳腺导管造影显示导管突然中断，有规则的充盈缺损者，多为乳头状瘤；若较大导管呈明显扩张，导管粗细不均匀，失去正常规则的树枝状外形者，则多为导管扩张综合征。必要时可行肿块针吸细胞学检查或活组织病理检查。<br />
