子宫内膜异位症的治疗应根据患者年龄、症状、病变部位以及对生育要求等不同情况加以全面考虑。原则上症状稍微者采用非手术疗法；有生育要求的轻度患者先行激素治疗，病变较重者行保守治疗；年轻无生育要求的重度患者可采用保留卵巢功能手术辅以激素治疗；症状和病变均严重的无生育要求患者可考虑根治性手术。<br />
(1)随访观察：适用于病变稍微、无症状或症状稍微患者，一般可每数月随访一次。对希望生育的患者，应作有关不孕的各项检查如输卵管通液试验或子宫输卵管碘油造影，非凡是在腹腔镜检查下行输卵管美蓝液通液试验，必要时解除输卵管粘连扭曲，以促使尽早受孕。若患者症状和体征加剧时，应改用其他较积极的方法。<br />
(2)性激素疗法：由于妊娠和闭经可避免发生痛经和经血逆流，并能导致异位内膜萎缩退化，因此，性激素已成为临床上治疗内膜异位症的主要非手术疗法，但对较大的卵巢子宫内膜异位囊肿，非凡是诊断尚未十分确定者或肝功能异常者均忌用性激素治疗。目前临床上采用的性激素疗法有：①假孕疗法；②高效孕激素疗法；③假绝经疗法；④雄激素；⑤促性腺激素释放激素激动剂；⑥18-甲基三烯炔诺酮。<br />
(3)中医治疗：中国历代中医学者在运用中医中药治疗该病中积累了丰富的经验，子宫内膜异位症属于中医痛经、症瘕、积聚、不孕的范畴，《诸病源侯论》曰：妇人月水来腹痛者，由劳伤气血以致体虚，风冷邪气客于胞络，损伤冲任之脉等。中医一般采用辨证施治手段，本着标本兼治为原则，温肾助阳，兼活血化瘀，调整机体的内环境，祛除产生该病的内因，同时温经活血祛瘀使月经正常来潮，痛经逐渐消失，异位包块、结节、巧克力囊肿即为中医所说的症瘕积聚溶解、吸收、消散。服药期间月经正常来潮，同时有促进卵泡生长及促排卵作用，故可受孕，受孕后对胚胎无任何不良影响，而受孕又是治疗该病的最佳方法之一。中医治疗子宫内膜异位症以其无副作用，疗效肯定在国际上处于领先水平，故治疗内异症中医是最佳选择，而且应及早治疗。<br />
(4)手术治疗：根据手术范围的不同，可分为保留生育功能、保留卵巢功能和根治性手术三类。<br />
