                        <p><font face="Arial" color="#000000">&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp; 子宫内膜异位症是较常见的一种妇科病，近年来它的发病有所上升。它的含义是由生长在&nbsp; 子宫腔以外的身体其他部位的&nbsp; 子宫内膜所引起的一种病变。卵巢是&nbsp; 子宫内膜异位症最常发生的部位，约占80%左右。其次是&nbsp; 子宫韧带及覆盖在&nbsp; 子宫、直肠或膀胱表面的盆腔腹膜等。</font></p>
<p><font face="Arial" color="#000000">　　卵巢巧克力囊肿 是&#8220;肿块&#8221;但并非是&#8220;肿瘤&#8221;，它是&nbsp; 子宫内膜异位症的一种病变。正常情况下，&nbsp; 子宫内膜生长在&nbsp; 子宫腔内，受体内女性激素的影响，每月脱落一次，形成月经。如果月经期脱落的&nbsp; 子宫内膜碎片，随经血逆流经输卵管进入盆腔，种植在卵巢表面或盆腔其他部位，形成异位囊肿，这种异位的&nbsp; 子宫内膜也受性激素的影响，随同月经周期反复脱落出血，如病变发生在卵巢上，每次月经期局部都有出血，使卵巢增大，形成内含陈旧性积血的囊肿，这种陈旧性血呈褐色，粘稠如糊状，似巧克力，故又称&#8220;巧克力囊肿&#8221;。这种囊肿可以逐渐增大，有时会在经期或经后发生破裂，但很少发生恶变。</font></p>
<p><font face="Arial" color="#000000">　　子宫内膜异位症的主要症状是痛经，它的痛经特点是渐进性痛经，是继发性和进行性加重。其次是不孕，和月经紊乱及性交痛等。它的发病主要原因与分娩，多次流产等有关，产后脱落的&nbsp; 子宫内膜种植在宫颈、阴道、盆腔内，以及反复的人工流产术，通过负压吸引至盆腔内。另外，妇科的某些小手术，如输卵管通气或通水，&nbsp; 子宫输卵管碘酒造影，宫颈电烫、冷冻等，都可能使月经期脱落的内膜在此种植，日后形成&nbsp; 子宫内膜异位症。</font></p>
<p><font face="Arial" color="#000000">　　其治疗原则是，对有生育要求的年轻妇女一般采用药物治疗并定期随访检查。若症状较重，经药物治疗效果不明显，或卵巢有较大包块的，可以采用保守性手术，如卵巢内膜囊肿巧克力液抽吸术，内膜囊肿剥除术或患侧卵巢切除术，以保留正常卵巢和生育功能。不过，经保守性手术后仍有可能复发。患者应坚持随访并争取尽早怀孕对40岁以上，年龄较大且无生育要求的妇女，可以切除全&nbsp; 子宫以及一侧或双侧卵巢和输卵管。</font></p>
<p><font face="Arial" color="#000000">　　正常妇女，在子宫的两侧，各有一拇指头大小的卵巢。卵巢虽小，但组织结构复杂，是保持女性正常的重要器官。卵巢肿物可发生在任何年龄，但多见于生育期妇女，大多数发生于卵巢功能最旺盛时期；其次为由旺盛转衰的时期，是妇科常见肿瘤，占女性生殖系统肿瘤的32%。近年，卵巢恶性肿瘤发病率增加2-3倍，呈逐渐上升趋势，占女性生殖器官恶性肿瘤的20%。对卵巢恶性肿瘤，目前无完善的早期诊断方法，晚期病例又疗效不佳，故其死亡率高居恶性肿瘤的首位，成为妇科恶性肿瘤中威胁最大的疾病。</font></p>
