<p> 子宫内膜异位症是目前比较常见的疾病，在女性不孕症中占有比较高的比例。该病之所以发生不孕，是由于异位的子宫内膜发生周期性出血，血液不能外流，刺激盆腔腹膜，发生粘连，形成疤痕，使输卵管受压梗阻；或使输卵管浆膜粘连成束、扭曲，从而影响摄取卵子的作用；或因粘连，子宫后倾固定，使精子不易进入子宫口；或因粘连包绕卵巢，卵子排不出；或因子宫内膜异位囊肿(又称巧克力囊肿)，异位于卵巢内的内膜病灶破坏了卵巢实质，影响排卵。</p>
<p> 另外，隐性子宫内膜异位症（主要指盆腔腹膜上散在着点状或小片状的异位子宫内膜，无症状，妇科检查也查不出来，只有通过腹腔镜才能发现的子宫内膜异位症），尽管没有上述种种原因，也会发生不孕症，这是因为异位的子宫内膜产生的前列腺素显著高于正常的子宫内膜，过高的前列腺素可阻止卵泡增长，影响输卵管的蠕动而影响受孕；还可促成黄体退化，易导致流产。异位内膜周期性出血，不能外流，机体巨噬细胞进行吞噬吸收，从而诱发自身免疫反应，如血清抗卵巢抗体、抗子宫内膜抗体升高，影响排卵和黄体功能，影响孕卵着床。部分子宫内膜异位症病人还可引起血清催乳素增高，抑制FSH分泌,使卵泡发育不良，导致不排卵或黄体功能不全而不孕。</p>
<p> 子宫内膜异位症导致女性不孕症发生的一个很重要的原因，就是异位的子宫内膜使输卵管受压梗阻、或使输卵管浆膜粘连成束、扭曲，从而影响到精子和卵子的结合。生长在子宫内的内膜组织之所以会到处游移，是由于外在性子宫内膜异位症最常发生的部位是卵巢、子宫骶骨韧带、盆腔膜腹等处，当剖腹取胎或剖宫产术时，子宫内膜组织可随宫内容物流出而污染腹壁伤口，并种植于其中，形成腹壁子宫内膜异位症，也属外在性子宫内膜异位症。异位的子宫内膜波及的部位较广，可侵入包绕盆腔的腹膜、悬吊子宫的韧带、子宫的近邻一—膀胱、直肠等处，以及大网膜、阑尾、小肠、腹壁切口，远至肝脏、胆囊、肾脏、肺脏、胸腔、四肢肌肉及骨骼等脏器。异位子宫内膜可出现在身体不同部位，但绝大多数位于盆腔内的器官，如卵巢，宫骶韧带，子宫下部后壁浆膜面以及覆盖直肠子宫陷凹,乙状结肠的腹膜层和阴道直肠隔，其中以侵犯卵巢者最常见，约占80%。其他例如宫颈，阴道，外阴亦有受波及者。此外，脐，输尿管，肺，胸膜，乳腺，淋巴结，甚至手，臂，大腿处均可发病，但极罕见。</p>
