<p> 附件肿块是有争议的腹腔镜手术之一，争议的焦点在于对卵巢癌，剖腹手术往往处理过度而腹腔镜则处理不够。术前必须进行细致的筛选，cAl25和B超，包括彩色超声检查都是必须的，只有在CAl25<35IU几和超声提示良性的病例，才考虑行腹腔镜手术。实性肿块一般不应在腹腔镜下手术。</p>
<p> 超声检查和cAl25检测卵巢肿瘤的敏感性和特异性不够理想，腹腔镜在这方面是满意的诊断工具，在有经验的医生手中，腹腔镜对于早期卵巢癌的诊断假阴性率<1％。</p>
<p> 另一方面，术前诊断卵巢肿瘤的假阳性结果并非罕见，这可能导致一些不必要的剖腹手术，腹腔镜用于决定是否有必要行剖腹手术或决定手术切口的形式，使患者从中受益。明显的恶性肿瘤手术必须经剖腹进行(纵切口)，而明显的良性肿瘤则可经剖腹(耻上横切口)或腹腔镜进行。</p>
<p> 如检查发现在卵巢的表面有乳头状或实质性肿瘤样结节生长，或者有可疑的腹膜转移灶时，就要怀疑有恶性的可能。当卵巢表面或腹膜正常时，卵巢癌的诊断并不能彻底排除，则须作活检作卵巢活检。</p>
<p> 当发现事先并没有怀疑的卵巢癌时，活检是有帮助的。目前主张作活检时使用剪刀而不是活检钳，原因是使用活检钳所取得的标本太小。但冰冻切片的诊断价值有时是有疑问的，此时，有4种选择，取决于是否有有经验的肿瘤医生，手术者腹腔镜的技术水平和从病人处得到准许的程度。</p>
<p> 第1种选择是咨询富有肿瘤手术经验的医生，行剖腹根治性手术。第2种是行活检，避免囊肿破裂并等待石蜡切片，以决定进一步治疗。第3种是施行附件切除术(腹腔镜或剖腹)以获取整个卵巢的病理诊断。第4种仅限于少数训练有素的医生，冰冻切片之后，行腹腔镜下手术，包括全子宫，双侧附件切除，盆腔或主动脉旁淋巴结摘除。</p>
