<P><FONT face=Arial color=#000000>&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp; &nbsp;一、LEEP的手术指征</FONT></P>
<P><FONT face=Arial color=#000000>　　文献提出LEEP的手术指征包括在细胞学和阴道镜下，(1) 怀疑CIN2,CIN3；(2) 怀疑宫颈早期浸润性癌或原位癌；(3) 持续CIN1或CIN1患者随访不方便；(4) 怀疑宫颈ASCUS或有症状的宫颈外翻等4种情况［1-3］。本研究结果支持此适应证。LEEP治疗存在的问题是切除过多组织，为慎重起见，本研究提出，≥ CIN2的行锥切术；ASCUS 和CIN1的行活检术。而传统电刀手术指征仅包括上文所述的(1)、(2)条件。</FONT></P>
<P><FONT face=Arial color=#000000>　　二、LEEP的手术范围</FONT></P>
<P><FONT face=Arial color=#000000>　　关于LEEP手术范围，根据文献报道病变≥2.5 cm 应锥切，锥切的范围应超过正常组织1 mm。根据二氧化碳激光治疗CIN的经验，发现深度影响治愈率，累及宫颈腺体的CIN平均深为1.24 mm，最深为5.22 mm［4］。激光治疗深度自3 mm增加到5 mm，病变治愈率也由68%提高到87%［5］。本研究提出，LEEP锥切宫颈理想深度为7 mm左右。又根据文献报道用一针式电极重复插入宫颈管深达1.5 cm 处，破坏颈管内病变［6］，这种改良提高了治愈率，使病变持续率降为2.7%。本研究提出LEEP锥切宫颈管理想深度为15 mm左右。结果提示，适用此范围的LEEP锥切术比传统电刀锥切术省时、省力、安全。对于LEEP行活检术，本研究结果证实宫颈深度4 mm、颈管深度4 mm，既可达到诊断和治疗作用，又避免切除过多组织，比宫颈钳活检术优越。</FONT></P>
<P><FONT face=Arial color=#000000>　　三、LEEP手术标本组织病理特点</FONT></P>
<P><FONT face=Arial color=#000000>&nbsp;&nbsp;&nbsp; 传统电刀组标本边缘百分之百有炭化现象，标本边缘无法确定是否切除干净，局灶性癌易丢失，因此对病理诊断有一定影响，对微小浸润癌的诊断造成困难。 LEEP术可提供完整的无炭化的组织标本，以确定病灶是否完全切除，并可确诊阴道镜或传统电刀术较难诊断的微小浸润癌，只要清楚认识热效应在细胞上引起变化的特点，则不妨碍病理结果的判别。</FONT></P>
