<p><font face="Arial" color="#000000"> 【发生率】</font></p>
<p><font face="Arial" color="#000000"> LUF的发生率因诊断方法和对象而有所不同，寺川报告不孕症患者60例384周期中，52例LUF周期(13.5%)。其中子宫内膜异位症占24.7%，盆腹膜炎18.3%，盆腔手术史占26.2%。国内郑怀美报告克罗米芬促排卵周期LUF约30%，指出这可能是该药促排卵治疗排卵率高而妊娠率低的原因。</font></p>
<p><font face="Arial" color="#000000"> 【病因】</font></p>
<p><font face="Arial" color="#000000"> 可能与以下两种因素有关：</font></p>
<p><font face="Arial" color="#000000"> 1.中枢性因素</font></p>
<p><font face="Arial" color="#000000"> 众所周知排卵前由于卵泡所分泌的大量雌二醇(250～300pg/ml以上)，对下丘脑和垂体的正反馈作用诱发了一个LH峰。LH峰的作用是促使卵泡开始第二次减数分裂;使颗粒细胞黄素化，通过前列腺素而使胶原酶活化，后者是排卵所需要的。如果LH峰不充分，则可使胶原酶活化受阻致排卵障碍，而颗粒细胞黄素化不充分，可使孕酮分泌减少。</font></p>
<p><font face="Arial" color="#000000"> 2.局部性因素</font></p>
<p><font face="Arial" color="#000000"> 卵巢局部的炎症影响卵巢血流和前列腺素代谢，使支持卵泡壁的胶原网状结构不能变薄，卵泡排不出，而继续增大。盆腔炎、盆腔粘连及盆腔内膜异位症病人的LUF的重复率较高。</font></p>
<p><font face="Arial" color="#000000"> 【诊断】</font></p>
<p><font face="Arial" color="#000000"> 1.超声波连续观测卵泡发育状况，结合基础体温上升，但卵泡不破裂反而继续增长，对诊断价值较大。LUF约</font></p>
<p><font face="Arial" color="#000000"> 60%在下次月经来潮前消失。</font></p>
<p><font face="Arial" color="#000000"> 2.腹腔镜检查</font></p>
<p><font face="Arial" color="#000000"> 相当于排卵时期作腹腔镜检查，如不能发现排卵孔可确定诊断，但排卵孔很容易经上皮化而修复，因而镜检假阳性率较高。</font></p>
<p><font face="Arial" color="#000000"> 3.在BBT双相，孕激素已上升但卵泡不排卵反倒继续增大时，可行卵泡穿刺术，如采得卵子可确定诊断，但是文献报告采卵率不高。</font></p>
<p><font face="Arial" color="#000000"> 4.排卵后腹水中的E2和甲浓度明显增加，而LUF没有变化。因其检查标准值设定较难，仅靠这一指标不能做出可靠诊断。</font></p>
<p><font face="Arial" color="#000000"> 【治疗】</font></p>
<p><font face="Arial" color="#000000"> 1.卵泡发育接近成熟时给予5000～10000UHCG可诱发排卵。</font></p>
<p><font face="Arial" color="#000000"> 2.前列腺素合成阻滞剂可诱发LUF，因而如消炎痛等非甾体抗炎剂的应用要限制。</font></p>
<p><font face="Arial" color="#000000"> 3.治疗易引起LUF的内膜异位症、盆腔炎症及粘连。</font></p>
<p><font face="Arial" color="#000000"> 4.LUF反复发作的病例可行GIFT或IVF-ET。<br />
