<p><font face="Arial" color="#000000"> 正常子宫腔的前后壁紧贴，但因内膜完整，不易发生粘连，即使在月经来潮内膜功能层剥脱，基底层仍完整，亦不会发生粘连。创伤（刮、吸宫）和继发感染是造成本征的主要原因，当然宫腔整形术后也是一种病因，综述如下： </font></p>
<p><font face="Arial" color="#000000"> </font></p>
<p><font face="Arial" color="#000000"> 1）损伤性刮宫：据文献报道，欧美11个国家298l例IUA中妊娠刮宫术后并发宫腔粘连者占91％，其中人流术后占66.7％，足月产后刮宫占21.5％，剖宫产术后占2％，葡萄胎刮宫后占6％，宫颈手术后占4.2％。 </font></p>
<p><font face="Arial" color="#000000"> </font></p>
<p><font face="Arial" color="#000000"> 2）感染：包括一般和特异性感染，如结核、血吸虫病、阿米巴和放线菌病等。</font></p>
<p><font face="Arial" color="#000000"> </font></p>
<p><font face="Arial" color="#000000"> 3）妇科手术损伤：如子宫肌瘤挖除术和畸形子宫矫治术后。</font></p>
<p><font face="Arial" color="#000000"> <br />
<br />
宫颈黏连症可导致不孕的原因在于： </font></p>
<p><font face="Arial" color="#000000"> </font></p>
<p><font face="Arial" color="#000000"> 1）损伤和感染破坏子宫内膜层完整性，引起宫壁组织疤痕粘连愈着而致宫腔闭锁，降低了子宫容受性。 </font></p>
<p><font face="Arial" color="#000000"> </font></p>
<p><font face="Arial" color="#000000"> 2）子宫内膜组织学变化：据研究观察表明宫腔粘连时子宫内膜组织相中呈分泌相者占80％，增生相12％，萎缩相5％，过度增生相3％；刮出物为子宫内膜者占65％，纤维组织25％，颈管内膜12．5％，内膜基底层6％，子宫平滑肌组织4％(Elgueta&Pena)。宫腔粘连内膜组织学上述改变不利于精子储存、成活和获能，也不利于孕卵着床、胎盘植入和胚胎发育。 <br />

据临床相关资料统计，宫腔粘连引起的不孕中各症状的发生率如下：</p>
<p><br />
1.原发性或继发性不孕占43％。</p>
<p><br />
2.月经失调闭经37％，稀发月经和月经稀少33％，痛经2．5％，月经过多1％，月经仍正常者6％。</p>
<p><br />
3.妊娠后并发症 习惯性流产14％，胎盘早剥1％，早产0．5％，前置胎盘0．1％。</p>
<p><br />
4.合并颈管粘连者偶可引起经血潴留，宫腔积血、积液或积脓。</p>
<p> </p>
<p> 宫腔粘连综合征发生粘连部位56%发生在子宫腔，24%影响到子宫腔和宫颈管，20%在子宫峡部。宫腔粘连使宫腔变形或输卵管开口处阻塞而致不孕，宫腔变形及子宫内膜血供不足，导致流产或早产。专家提醒育龄期女性需洁身自爱，尽量避免人为因素造成的宫腔粘连。<br />
