                        <p><font face="Arial" color="#000000">&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp; 宫颈异常引起的不孕，一般不被人们所重视。据统计约占不孕因素的20%。子宫颈作为精子通过的第一道关隘，其解剖生理上的任何改变均可以影响精子的通过。</font></p>
<p><font face="Arial" color="#000000">　　宫颈异分多种情况, 其病因及临床表现具体如下.</font></p>
<p><font face="Arial" color="#000000">　　1、宫颈管闭锁与狭窄 先天性宫颈管闭锁或狭窄多由于双侧苗勒氏管下段形成和融合不全所致。临床上罕见，常伴有子宫发育不全，第二性征大多发育正常。如患者子宫内膜功能良好时，青春期可因宫腔积血而出现周期性下腹痛，或月经过少伴痛经，经血还可经输卵管逆流入腹腔，引起盆腔子宫内膜异位症。</font></p>
<p><font face="Arial" color="#000000">　　后天性宫颈管闭锁与狭窄多见于人工流产后或宫颈炎电灼、冷冻治疗后。主要原因是吸宫时宫颈扩张不充分，或带着负压取出吸管，造成宫颈内膜损伤，创面粘连闭锁。 　　宫颈炎治疗电灼或冷冻过深导致宫颈管粘连或狭窄。临床表现为人工流产后或宫颈炎治疗后出现闭经伴有周期性下腹痛。妇科检查发现阴道呈紫蓝色，宫颈举痛明显，宫体稍饱满、活动，有压痛。</font></p>
<p><font face="Arial" color="#000000">　　2、宫颈管发育不良 可伴子宫发育不良。严重发育不良的宫颈呈细长形。中度发育不良的宫颈长度与宫体之比为1：1，即所谓"青春型子宫"。宫颈发育不良可导致宫颈腺体分泌功能不足。</font></p>
<p><font face="Arial" color="#000000">　　3、宫颈管位置异常 常伴有子宫体的位置异常。慢性盆腔炎或子宫内膜异位症等可引起子宫极度后倾、后屈或前屈，使宫颈外口贴向前穹窿，致使后穹窿变浅而失去了贮精池的作用，从而不利于精子的上行。此外宫颈延长、过短或宫颈脱垂亦可能改变了宫颈外口与后穹窿之间的正常的位置关系，妨碍精子上行。 　　4、宫颈肌瘤 宫颈肌瘤造成不孕原因主要是颈管发生变形、狭窄，影响精子通过。临床表现主要为月经不规则，经血量增多，白带增多或膀胱、直肠症状。部分患者无症状。妇科检查可发现宫颈局部有突出肌瘤结节或子宫颈外形发生改变，肌瘤所在一侧宫颈扩大，而对侧被压变薄，宫颈外口伸张展平呈麻花形。</font></p>
<p><font face="Arial" color="#000000">　　5、慢性宫颈炎 是妇科常见病，一般不影响受孕。正常宫颈粘液能保护精子，供给能量，并且是贮存精子的场所。临床表现主要症状是白带增多。由于病原菌、炎症的范围及程度不同，白带的量、性质、颜色及气味也不同，可呈乳白色粘液状，有时呈淡黄色脓性、伴有息肉形成时易有血性白带或性交时出血。当炎症经子宫骶韧带扩散至盆腔时，可有要、骶部疼痛、盆腔部下坠痛及痛经等，每于月经、排便或性交时加重。检查时可见宫颈有不同程度的糜烂、肥大，有时变硬，见息肉、裂伤、外翻及腺体囊肿等病变。</font></p>
<p><font face="Arial" color="#000000">　　6、宫颈粘液异常 宫颈粘液中所含的葡萄糖及其它营养物质对穿越宫颈时的精子的生存和活动力有很大影响。因为精子本身仅储存少量糖原，在它停留及穿越女性生殖道时必须依靠细胞外的营养物质来满足自身能量的需要。卵巢激素调节宫颈粘液的分泌，宫颈粘液的物理特性及某些化学组成呈周期性改变以利于精子的穿透、营养及生存。</font></p>
