盆腔瘀血症的诊断参考</P>
<P> 盆腔瘀血症的症状涉及范围极广，而体征上又与某些其他病变混同，因此常造成临床诊断上的困难。但如能详细询问病史，注意症状与体征上的差异，除外其他有关疾病，适当应用盆腔静脉静脉造影及腹腔镜检查，即可作出较为可靠的诊断。以下几点可以提供参考： </P>
<P> 一、病人多为生育年龄妇女，有过两次或两次以上的生产、流产史，继某次生产或流产后不久，出现上述慢性盆腔疼痛，低位腰痛、性感不快、痛经等症状，但并无产后或流产后感染史。</P>
<P> 二、自觉症状与客观检查所见不相适应。病人诉说的症状多而严重，而妇科检查仅见宫颈肥大、紫蓝、有的有糜烂，子宫后倒于骶骨凹内，但如猛一触动子宫颈或触抵后穹窿，就会引起相当严重的盆腔及腰骶部疼痛，宫旁及附件区有明显的压痛及饱满感，如继续慢慢地触摸时则有一种软如海绵的感觉，并没有慢性附件炎所常有的增厚及硬的条索状物，也没有腹肌紧张及反跳痛。当企图用手法使后倒的子宫体复成前位时，病人更是感到疼痛难忍。</P>
<P> 三、常伴有经前期乳房肿痛及某些神经衰弱的症状。</P>
<P> 四、盆腔瘀血症虽曾被诊断为“慢性附件炎”或“慢性盆腔炎”，但却很少妨碍怀孕。甚至于人工流产后被诊断为慢性附件炎，症状尚未治愈就又怀孕了。</P>
<P> 五、对以往按慢性盆腔为所进行的各种治疗均收效不大，甚至毫无效果。病人自觉有严重的妇科病，久治不愈，被妇科医生视为难治之症。</P>
<P> 六、对于具有前述症状，经妇科检查及其他辅助诊断方法，如子宫输卵管造影等，排除盆腔器质性病变，而临床疑为盆腔静脉瘀血症，可行盆腔静脉造影协助诊断，具体实施详后。</P>
<P> 七、盆腔静脉造影术 盆腔静脉造影术是将造影剂注射在子宫腔底部肌层内，使子宫静脉、卵巢静脉及部分阴道静脉，髂内静脉显影，并以一定时间之间隔连续拍片，了解盆腔血液(主要是子宫静脉及卵巢静脉)流出盆腔的时间，作为辅助诊断瘀血症的一个方法。</P>
<P> 在盆腔静脉血运正常时，造影剂通常在20秒内完全流出盆腔；而在盆腔瘀血症时，静脉回流速度明显变慢，造影剂流出盆腔，要20秒以上的时间。</P> </P></SPAN>
