<p><font face="Arial" color="#000000"> 黄体功能不全(corpus luteum insuffiiency)是指黄体分泌的雌、孕激素不足，子宫内膜的分泌性变化不充分。黄体是卵巢排卵后形成的，主要功能就是分泌孕激素和一定量的雌激素。黄体功能不全常导致黄体期出血、孕卵着床障碍、不孕、习惯流产。</font></p>
<p><font face="Arial" color="#000000"> 【病因】</font></p>
<p><font face="Arial" color="#000000"> 正常黄体功能的维持有赖于下丘脑—垂体—卵巢性腺轴功能的完善，不仅黄体期，卵泡期出现异常也可致黄体功能障碍。目前一般认为黄体功能不全与下列因素有关：</font></p>
<p><font face="Arial" color="#000000"> 1.卵泡期FSH分泌不足，卵泡液中FSH和雌二醇低值。</font></p>
<p><font face="Arial" color="#000000"> 2.排卵期LH峰不充分。</font></p>
<p><font face="Arial" color="#000000"> 3.黄体期LH分泌不足或其脉冲式分泌不充分。</font></p>
<p><font face="Arial" color="#000000"> 4.子宫内膜细胞甾体激素受体异常，对黄体分泌的激素反应性低下，即使黄体功能正常，内膜发育不良。</font></p>
<p><font face="Arial" color="#000000"> 【诊断】</font></p>
<p><font face="Arial" color="#000000"> 1.基础体温 是评价黄体功能最简便常用的方法。黄体分泌的孕激素作用于下丘脑的体温中枢形成高温相，体温上升所需要的孕酮浓度为2.5ng/ml以上。高温相与低温相的差在0.3℃以上。基础体温高温相不足10天，高温相缓慢上升、高温相中间有陷落等都提示黄体功能障碍。</font></p>
<p><font face="Arial" color="#000000"> 2.血中孕酮浓度 血中孕酮浓度的测定是判断黄体功能的重要可靠指标。5ng/ml以上有排卵，黄体中期10ng/ml以上黄体功能正常。不足10ng/ml可诊断黄体功能不全。</font></p>
<p><font face="Arial" color="#000000"> 3.子宫内膜组织像 子宫内膜的采取时间多主张在黄体期中期即高温相第7天进行。此时恰为胚泡着床时间。也有人主张在黄体期后期即高温相第11—13d取材，中期主要观察腺上皮变化，后期则主要观间质细胞变化。如果内膜组织像与基础体温有2d以上差距的话，可诊断黄体功能不全。</font></p>
<p><font face="Arial" color="#000000"> 现在发现一些临床诊断为黄体功能不全的患者，经腹腔镜检查后发现是未破裂卵泡综合征。可见黄体功能不全还需要结合B超和腹腔镜检查。<br />
