              妇女下腹部疼痛有哪些原因？</P>
<P>　　妇女下腹部疼痛可发生急性腹痛和慢性腹痛两种。 <BR>&nbsp;</P>
<P>　　(1)急性下腹部疼痛可由下开原因引起：盆腔脏器的急性炎症，如子宫附件炎，局限性肠炎；盆，腹部脏器的穿孔或破裂，如异位妊娠，卵巢囊肿破裂，胃，十二指肠溃疡，胃癌，阑尾炎，胆囊穿孔以及阑尾脓肿破裂，其中以异位妊娠破裂及阑尾穿孔脓肿常见。</P>
<P>　　(2)还有急性肠梗阻，腹部脏器的结石，盆腔部脏器的急性血运障碍，如卵巢瘤蒂扭转，输卵管积水扭转，肠扭转等，以及寄生虫，痛经，盆腹部外伤均可引起急性下腹部疼痛。 有的腹痛就在病灶部位或其附近，有的则与病灶部位不符合，并且随着病情的发展，腹痛的部位又有转变。</P>
<P>　　腹痛又分为两种类型：</P>
<P>　　(1)痛觉所在部位即在病变脏器本身的脏器痛。</P>
<P>　　(2)向体壁放散的牵涉痛。前者亦可转为后者。妇科方面的急腹症多表现为脏器痛，如卵巢瘤蒂扭转，输卵管积水扭转。但也有先感觉腹痛在患部，以后病情发展又有变化者，所以应特别重视牵涉性疼痛。</P>
<P>　　慢性腹痛以下腹钝痛及骶部疼痛为主。多为慢性子宫颈炎，慢性附件炎，慢性盆腔结缔组织炎，盆腔淤血症，子宫后位，子宫肥大症，子宫脱垂等所致。下腹钝痛，下坠感，腰酸等症状多在劳累后，久站，性交后及月经期加重。 相当一大部分慢性腰部疼痛与妇科病无关，例如骶骼关节劳损，腰肌劳损，椎间盘脱出及其他病变。应注意疼痛起始时间，强度，持续或间断情况，疼痛开始部位以后有无转变，扩大，拒按还是喜按，有无合并症状，如畏寒，发烧，恶心，呕吐，便秘，排气多，停经或月经紊乱及有无包块等。 腹部肌肉紧张，抵抗，压缩者多为炎症或出血所致：松弛者多系子宫肌肉收缩或泌尿系统病变。痛觉不仅取决于病变的刺激，还决定于机体的神经精神状态。</P>
