<p><font face="Arial" color="#000000"> 宫腔镜应用于临床，对于有刮宫史，月经量减少的可疑病人行宫腔镜检查，融诊断与治疗于一体，一些较难处理的妇科疾病能直观、简单、安全地解决。对于膜性粘连、纤维肌性粘连可在宫腔镜下分离或用手术剪除；而对于结缔组织样致密粘连则需在B超或腹腔镜监护下行电切分离术，术后放置宫内节育器或蛋白胶防止再粘连，使病人恢复月经来潮，达到生育目的。</font></p>
<p><font face="Arial" color="#000000">宫腔粘连是由于近期妊娠子宫损伤后瘢痕所致，大约９０％的病例因刮宫所引起。通常损伤发生在足月分娩、早产、流产后１—４周，多量阴道出血而需刮宫时。宫腔粘连通常可致月经异常，如月经过少，严重粘连可引起闭经。若粘连封闭部分宫腔，患者可能怀孕，但易发生流产、早产、异位妊娠、胎死宫内、胎盘植入、胎盘粘连等，如完全闭锁患者可表现为不孕。宫腔粘连可分为内膜性粘连、纤维肌性粘连和结缔组织性粘连。</font></p>
<p><font face="Arial" color="#000000"></font> </p>
<p><font face="Arial" color="#000000">宫腔镜没有出现以前用输卵管碘油造影或B超诊断宫腔粘连，易漏诊，且不能提示粘连的坚韧度和粘连的类型，所以只有宫腔镜才能最后确诊。对于有刮宫史，月经量减少的可疑病人应行宫腔镜检查，不但可以判断粘连的程度、粘连的类型，且可以判断粘连的坚韧度。宫腔粘连患者宫腔镜下可见膜样、网状或纤维索状粘连，周边型粘连的宫腔周边有粘连索或周边瘢痕化，宫腔变窄呈新月状或桶状，从内口向内看宫腔不对称，一侧或两侧宫角看不到。</font></p>
<p><font face="Arial" color="#000000">宫腔镜，融诊断与治疗于一体，一些较难处理的妇科疾病能直观、简单、安全地解决。对于膜性粘连、纤维肌性粘连可在宫腔镜下分离或用手术剪除；而对于结缔组织样致密粘连则需在B超或腹腔镜监护下行电切分离术，术后放置宫内节育器或蛋白胶防止再粘连，使病人恢复月经来潮，达到生育目的。</font></p>
<p><font face="Arial" color="#000000">宫腔镜检查宫腔粘连分类：</font></p>
<p><font face="Arial" color="#000000">依据组织可以分为：可分为膜性粘连、结缔组织性粘连和肌性粘连三种。</font></p>
<p><font face="Arial" color="#000000">依据面积可以分为5度（欧洲妇科内镜协会分类）：I度：宫腔内多处有纤维膜样粘连带，两侧宫角及输卵管开口正常；II度：子宫前后壁间有致密的纤维素粘连，两侧宫角及输卵管开口可见；III度：纤维索状粘连致部分宫腔及一侧宫角闭锁；IV度：纤维索状粘连致部分宫腔及两侧宫角闭锁；Va度：粘连带瘢痕化致宫腔极度变形及狭窄；Vb度：粘连带瘢痕化致宫腔完全消失。</font></p>
<p><font face="Arial" color="#000000">宫腔镜对于膜性粘连、纤维肌性粘连可在宫腔镜下分离或用手术剪剪除；而对于结缔组织样致密粘连则需在Ｂ超监护下行电切分离术，术后放置宫内节育器，水囊防再粘连，并给予雌孕激素续贯用药，促使内膜生长。使病人恢复月经来潮，有的病人可以再次怀孕。安太医院院自开诊以来，诊治病人千余人次，取得满意临床效果。</font></p>
