                        <p><font face="Arial" color="#000000"></font>&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp; <font face="Arial" color="#000000">外阴、阴道疾病引起的不孕占不孕症的1％～5％。阴道是性交和精液的容受器，某些外阴、阴道器质性或功能性疾病影响了精液或精子进入并储存在阴道内，或由于其环境变化影响了正常精子的功能而致不孕。</font></p>
<p><font face="Arial" color="#000000">　　先天性无阴道</font></p>
<p><font face="Arial" color="#000000">　　在胚胎期约7-10周性器官分化形成中，两侧副中肾管会合后，其尾端发育受阻或停滞未能向下发展，而致先先天性无阴道，常合并先天性无子宫或始基子宫，偶有正常发育的子宫。卵巢来源于生殖嵴，很少同时受累，通常发育与功能正常。先天性无阴道是Rokitansdy-Kuster-Hausersyndrome(R-K-H综合征)的临床表现之一。</font></p>
<p><font face="Arial" color="#000000">　　【临床表现】原发性闭经，亦有周期性下腹痛者，婚后不能性交等。检查女性表型，外阴正常，处女膜内有浅窝无阴道；无子宫或始基子宫畸形，可能合并泌尿系统或骨骼畸形。卵巢功能测定属正常范围。</font></p>
<p><font face="Arial" color="#000000">　　【诊断与鉴别诊断】根据病史、妇科检查，B超扫描盆腔与肾区、内分泌测定、染色体核型分析或PCR技术检测性基因可确诊。同时应作X线泌尿系统造影确定有无畸形，骨骼摄片有否畸形。</font></p>
<p><font face="Arial" color="#000000">　　应与继发性阴道狭窄或闭锁相鉴别</font></p>
<p><font face="Arial" color="#000000">　　【治疗】根据患者子宫发育情况，决定治疗时机与方法。</font></p>
<p><font face="Arial" color="#000000">　　1.治疗时机　经详细检查患者有近正常的的子宫体、卵巢功能正常，则应在月经初潮之后选期手术，以利经血引流、性交，经治疗有希望宫内妊娠。若无子宫或始基子宫等无生育可能者，最好在婚前后选期行人工阴道成形术，主要解决性生活问题。</font></p>
<p><font face="Arial" color="#000000">　　2.方法</font></p>
<p><font face="Arial" color="#000000">1）阴道成形术：术式较多，例如小阴唇皮瓣阴道成形术，大腿内侧皮片阴道成形术、乙状结肠代阴道成形术，腹膜代阴道成形术等。具体方法参考妇科手术学。</font></p>
<p><font face="Arial" color="#000000">　　术后一旦妊娠应重点监护，近预产期应选期剖宫产。</font></p>
<p><font face="Arial" color="#000000">　　（2）顶压法：对无生育可能，有部分盲袋样阴道，根据患者意愿，可试用局部顶压法，用如试管底部样圆钝的模具长期自行顶压，性生活亦可逐渐加深，若不满意也为手术创造了较好条件。</font></p>
