子宫肌瘤是女性生殖器官中最常见的一种良性肿瘤，不孕妇女尤为多见。子宫肌瘤患者的不孕率约为30-40%，比一般人群不孕率（10%）为高。
<p> 子宫肌瘤影响受孕能力的程度，与肌瘤的部位、大小和数目有关。为此，有必要向读者介绍一下子宫肌瘤导致不孕可有的几种情况，子宫肌瘤由子宫肌层长出，开始时它生长在子宫肌壁内，然后肿瘤不断增大，向不同方向生长，大体为下述三种情况：</p>
<p> 第一，肌瘤向子宫表面发展，突出于子宫表面，称为浆膜下子宫肌瘤，常可由一个蒂与子宫表面连着，这类肌瘤对受孕影响较小，手术切除也容易，但长到一定大小时，会影响分娩的顺利进行。</p>
<p> 第二，肌瘤朝着子宫腔的方向发展，突出在子宫腔内，有时还会突出到子宫颈外口，可在阴道内发现，这类肌瘤称为粘膜下肌瘤，引起的症状较多，常见为月经增多及阴道流水并带臭味。当粘膜下肌瘤长在输卵管内口附近或长在子宫腔上半部时，往往影响精子通行，不能使受精卵坐胎和发育。这类肌瘤可通过阴道检查、诊断性刮宫、子宫造影或Ｂ型超声确诊，凡确诊为粘膜下肌瘤的都应手术摘除。一般不需开腹，从阴道经宫颈就可将瘤子切除。</p>
<p> 第三，肌瘤局限在子宫肌壁内，称作壁间肌瘤，约占肌瘤的60-70%。它不仅能影响受孕而且常引起流产或早产。通常，这三类肌瘤混合生长成多发性肌瘤或兼而有之，较少为单个性的。</p>
<p> 除此之外，从部位上分，肌瘤可长在子宫底部，也可长在子宫颈部，然而绝大部分是长在子宫体部。综上所述，子宫肌瘤引起不孕的原因，常是由于肌瘤正好堵住子宫颈口或输卵管内口而影响精子通行，或肌瘤占据了受精卵着床的部位而不能坐胎。即使受孕，也常因影响胚胎发育而流产。至于到了妊娠晚期，可由于子宫收缩力的异常，而引起早产、阻碍分娩或造成产后大出血。因此，不要认为子宫肌瘤最常见而轻视它，也不要因为经过几次流产而失去治疗信心。总之，子宫肌瘤引起的不孕或不育，并非不治之症。它不仅容易诊断，而且手术治疗的效果也是很好的。不孕妇女的肌瘤切除后，可明显提高妊娠率。<br />
