创建一个符合iso-9660标准的iso文件系统

Modified version of libhfs (v2.0) to work with mkhybrid.

To complile for mkhybrid, use the #define -DAPPLE_HYB

The libhfs.a created with this option must not be used with other
hfsutils routines.
