Such programs could help prevent cognitive errorsin health care, bring medical expertise to underservedparts of the world, and improve medical education andtraining. Accordingly, the Laboratory's Clinical DecisionMaking Group seeks to learn how doctors think about thediagnosis and treatment of various medical conditions, andhow their approaches may be captured by AI methods.
The greatest challenge in building computer programsdesigned to mimic medical reasoning is that they must dealwith many complexities. Diseases may appear in unusualcombinations, for example, and exhibit variable symptoms.Medical care also is rife with basic uncertainties becauseof our incomplete understanding of disease and because ofnormal variations among patients. Further complicating theissues, AI programs must reflect individual patient valuesand preferences regarding decisions about potentially riskydiagnostics and treatments.
Working with physicians from nearby institutions (includingNew England Medical Center and Children's Hospital), we areexploring the ways in which medical knowledge isrepresented, and how disease may be explained asphysiological "derangements" of the healthy individual.Theoretical efforts develop general techniques forrepresenting physiological, anatomical, and biochemicalknowledge; exploring the consequences of diseasepathologies; evaluating time-dependent data; and elicitingand representing patient preferences. Empirical studiesinvolve implementation of computer programs that solvespecific medical challenges. Evaluation of the capabilitiesand usefulness of such programs helps lead to a betterunderstanding of future research needs.
Specific projects within the Group are selected based onthe clinical importance of various disorders and on theirusefulness in designing new AI-related technologies.Currently we are working on applications to <!WA6><AHREF="http://www.medg.lcs.mit.edu/projects/hf.html">congestiveheart failure, <!WA7>geneticcounseling, the detection of abnormal growth in children, and monitoring treatment inthe intensive care unit (ICU). Earlier studies include thediagnosis and treatment of coronary artery disease, fluidand electrolyte disorders, and Hodgkin's disease (asometimes fatal disease of the lymphatic system). We alsohave studied the design of clinical trial protocols forlung cancer therapies; problems in administering digitalis;diagnosis of the causes of acute renal failure and otherkidney disorders, and the management of ventriculararrhythmias (irregular heart function) in the cardiac ICU.

<!WA8><!WA9><!WA10><!WA11><!WA12><!WA13>
