  Here is our release Rev 1.0 of the DC550 Jack Board. The attached zip file contains: 

    DC550_JACK_BOARD_V1_0.DSN - OrCad schematic database
    DC550_JACK_BOARD_V1_0_NET.TXT - netlist in Pads2K format 
    DC550_JACK_BOARD_V1_0_BOM.TXT - BOM in text format 
    DC550_JACK_BOARD_V1_0_XRF.TXT - cross reference file output by OrCad for your convenience
    and the PDF version of the schematic in 3 separate PDF files.

  There have been a few changes since the first version of the Jack Board:

  - swap the connections to pins 2 and 3 of the phone jack J1
  - remove the varistor D2
  - replace the 100V surge protector D3 with a stronger part (Digikey 5KP70ACCCT-ND) 
  - add a new 1N4004 diode (called D2) to protect against reverse surges
  - rename nets on pins 6 and 7 of J2 to match Main Board
  - add alternate resettable fuse F2 (Digikey TR150-ND)

  I think the parts placement can remain almost the same as the first version of the Jack Board, with the new diode squeezed in. 

  Put the footprint for F2 on top of the footprint for F1.  This will provide a stuffing option for the alternate fuse.
