There are lots of tools (probably a dozen or more) for help authoring
and they are discussed in great detail over in the .winhelp group.
Personally, I don't think you need any special tool if you have
Microsoft Word, particularly if you're creating help for your app.
The help authoring tools are favoured more by non-programmers perhaps,
i.e. those who are doing standalone desktop publishing using .HLP
files). They seem to be quite concerned with the fancy things like
popup windows and hot-spot bitmaps and external DLLs. I guess the
HA tools are useful for those things.
But for simple .HLP for an application, just write the text, put
in a few footnotes & underscores, save as RTF and compile. It's
really quite simple:
(a) separate topics with a hard page break
(b) single underscore for popup links
(c) double underscore for goto links
(d) hidden text for the 'goto' part for (b) & (c)
(e) # footnotes for the label of the topic
$ footnotes for topic titles
K footnotes for keyword lists for each topic
+ footnotes for browse sequences

Seconded. ForeHelp is not free but is excellent value. You will
probably also want to get the protected mode help compiler
HCP.EXE, available for download from Microsoft (I believe).

I have found Visual Help and finally paid the shareware fee and have
no regrets. The tool is nice, the manual is good and I was able to design
great help files.
I don't remember the price, but it was something like 40-50$. You also
can buy a more professional version which allows more presentation tools
and bolds, italics, ...
